Why we built this pack

If you have ever started a loan or grant application, the first hour goes to hunting for the right blank forms — and half the copies online are outdated or locked behind a third-party site. This pack collects the current funding application forms in one place and hands them to you as editable Word and Excel files, so you can read every question, type your answers, let the budgets add themselves up, and keep a clean draft for your files.

It is a preparation tool. Lenders and grant portals still want the final version submitted on the official form or through their own system — your bank’s portal, or Grants.gov Workspace — but drafting from an editable copy first makes that step fast instead of stressful.

These Word and Excel files are approximate reconstructions of the official forms, for drafting only — they are not official filing copies. Government forms also change often, so open the source link for each one and confirm the current edition before you file. This pack is information, not legal, tax or financial advice.

Everything in the pack, folder by folder

01 · SBA business loans (7(a) and 504)

Eight documents for the two main SBA-guaranteed business loan programs, including the forms every borrower completes and the lender’s own submission checklist.

  • SBA Form 1919 — Borrower Information Form (Word, edition 02/2025). The core borrower questionnaire for every 7(a) and 504 loan: the business, its owners, the loan request and eligibility questions.
  • SBA Form 413 — Personal Financial Statement (Excel, edition 05-24). Assets, liabilities and net worth for each owner and guarantor. Total assets, total liabilities and net worth calculate automatically.
  • SBA Form 2202 — Schedule of Liabilities (Excel). An itemised list of the business’s existing debts; column totals calculate automatically.
  • SBA Form 159 — Fee Disclosure and Compensation Agreement (Word). Required whenever a packager, agent or broker is paid to help with the application.
  • SBA Form 912 — Statement of Personal History (Word). The character declaration, filed when a background question on Form 1919 is answered “yes.”
  • SBA Form 1244 — Application for Section 504 Loans (Word, edition 11-2022). The project application for a CDC / 504 fixed-asset loan (real estate and equipment).
  • SBA Form 1010-Business — Representative disclosure (Word). Filed with an 8(a) Business Development application when a paid representative helped prepare it.
  • SBA 7(a) Loan Submission Checklist, v2.0 (Word). The full document list a lender assembles for a 7(a) file — a ready-made prep checklist for you.

02 · SBA disaster loans

The application set for low-interest SBA disaster loans after a federally declared disaster — for businesses, homeowners and renters.

  • SBA Form 5 — Disaster Business Loan Application (Word, edition 05/23). Covers both physical damage and economic injury for businesses and most private non-profits.
  • SBA Form 5C — Disaster Home / Sole Proprietor Loan Application (Word, edition 05/23). For homeowners, renters and sole proprietors.
  • SBA Form 2202 — Schedule of Liabilities (Excel). Filed together with Form 5; a copy is included here for convenience.
  • SBA pre-filled IRS Form 4506-C, 2025 tax year (Word). SBA’s own version of the tax-transcript release for disaster-loan applicants.

03 · IRS tax forms

The IRS forms that come up alongside almost any loan or grant: tax-transcript authorisation, an EIN application and a taxpayer-ID certification. All Word.

  • IRS Form 4506-C — IVES Request for Transcript of Tax Return (Rev. 10-2022). Authorises the IRS to send your filed returns to a lender for income verification. Standard in SBA and mortgage underwriting.
  • IRS Form 4506-T — Request for Transcript of Tax Return. The older transcript-request form; some programs still use it.
  • IRS Form SS-4 — Application for Employer Identification Number (Rev. December 2025). Get an EIN for a business, non-profit, trust or estate.
  • IRS Form W-9 — Request for Taxpayer Identification Number and Certification (Rev. March 2024). Give your TIN to a grant office, client or lender that must report payments to you.

04 · Federal grants (Grants.gov standard forms)

Seventeen files covering the government-wide SF-424 form families. The official versions are dynamic PDFs, so these Word and Excel copies are the roughest in the pack — use them to see what is asked, read the bundled official Instructions, and complete the live forms in Grants.gov Workspace.

  • SF-424 — Application for Federal Assistance, v4.0 (Word), plus the official completion instructions. The core cover form for discretionary grants.
  • SF-424A — Budget Information, Non-Construction (Excel), plus instructions. Sections A–F; section totals, line 6i / 6k and column totals calculate automatically.
  • SF-424B — Assurances, Non-Construction (Word). The standard applicant certifications.
  • SF-424C — Budget Information, Construction (Excel). Cost-classification budget; subtotals, total project cost and the allowable-cost column calculate automatically.
  • SF-424D — Assurances, Construction (Word). Certifications for construction projects.
  • SF-LLL — Disclosure of Lobbying Activities (Word). Required when lobbying funds are involved.
  • SF-425 — Federal Financial Report, versions 1.0 and 2.0 (Excel). Lines 10c, 10g, 10h, 10k and 10o calculate automatically. Both published versions included.
  • SF-424 (Mandatory) v3.0 (Word). The cover form for formula, block and other mandatory grants.
  • SF-424 (Individual) v2.0 (Word). The cover form when the applicant is a person, not an organisation.
  • SF-424 (R&R) v4.0 (Word) and R&R Budget v1.4 (Excel). The Research & Related forms used by NIH, NSF and similar agencies; the budget totals each section automatically.
  • Project / Performance Site Location(s) form (Word). Where the work will be carried out.
  • Project Narrative Attachment form (Word). The wrapper for your narrative upload.
  • Budget Narrative Attachment form (Word). The wrapper for your budget justification upload.

05 · Home mortgage loans (URLA / Form 1003)

The redesigned Uniform Residential Loan Application — Fannie Mae Form 1003 / Freddie Mac Form 65, effective 1/2021 — used for conventional, FHA and VA home loans, with every component. All Word.

  • URLA — Borrower Information. The main application: personal details, employment, income, assets, liabilities and declarations. Required on every loan.
  • URLA — Lender Loan Information. The lender-completed companion section. Required on every loan.
  • URLA — Additional Borrower. For a second or later borrower on the same loan.
  • URLA — Unmarried Addendum. Used in certain states to capture the rights of an unmarried co-applicant.
  • URLA — Continuation Sheet. Extra space for any section that overflows.
  • URLA — Instructions. Fannie Mae’s official field-by-field guide.
  • Supplemental Consumer Information Form (SCIF / Form 1103, 5/2022). Captures homeownership-education, housing-counseling and language-preference data. Mandatory for conventional loans sold to the GSEs since March 1, 2023.

06 · USDA Rural Development loans and grants

Application forms for USDA Rural Development, which backs both rural business loans and rural home loans. All Word.

  • Form RD 4279-1 — Application for Loan Guarantee. The Business & Industry / OneRD guaranteed-loan application for rural businesses.
  • Form RD 410-4 — Uniform Residential Loan Application, Rural Housing (Rev. 05-25). Used for Section 502 Direct rural home loans; guaranteed rural home loans use the standard URLA in folder 05.
  • SF-424 — Application for Federal Assistance (USDA copy). The cover form for USDA Rural Development grant programs.

07 · Business plan and financial templates

Starter templates from the SBA’s own “Write your business plan” guide — the supporting documents a lender or grant reviewer usually asks for next.

  • SBA traditional-format business plan example (Word). A complete worked example, “We Can Do It Consulting,” you can overwrite section by section.
  • SBA lean-format business plan example (Word). The one-page “Wooden Grain Toy Company” format for a faster first draft.
  • SBA Startup Costs Worksheet (Excel). One-time and monthly costs; section totals and “Total funds required” calculate automatically.

Readme.txt - a plain-text index that lists, for every file: the official source page, the direct link, the edition or version, and the OMB control number. Each link was checked and returned a working page in September 2026.

Where the files come from

Every file was rebuilt from the official government PDF published by the agency or GSE that owns it:

  • U.S. Small Business Administration — sba.gov and its document library at legacy.sba.gov
  • Internal Revenue Service — irs.gov
  • Grants.gov — the federal grant portal’s forms repository
  • Fannie Mae — the Single-Family Uniform Residential Loan Application page
  • USDA Rural Development — rd.usda.gov and the USDA eForms system

Nothing was sourced from a third-party form site. Because these are editable reconstructions rather than the government PDFs themselves, the layout is approximate and the wording should be checked against the official form — every source link is in the Readme, together with the edition or version number and, where the form shows one, the OMB expiration date. A couple of SBA forms currently carry an OMB date that has already passed; that reflects a pending federal paperwork renewal on the SBA’s side, and the SBA still publishes those editions as current. The Readme flags them.

Draft here in Word and Excel. File on the official form or portal when you’re ready.

What is not in the pack

Some funding routes have no single official blank form. For those, the Readme points you to the right starting place instead:

  • State and city grant, loan and incentive programs. Each runs its own portal and forms — start with your state’s economic-development agency.
  • SBA Microloans. Applications are handled by each nonprofit intermediary lender using its own form.
  • Consumer credit cards and personal loans. Every bank uses its own application; there is no government blank.
  • Mortgage disclosures you receive rather than fill in — the Loan Estimate and Closing Disclosure. Your lender generates these.
  • SAM.gov entity registration. Required before any federal grant, but it is an online process, not a downloadable form.
  • SBA Form 1368 (the EIDL monthly-sales schedule), which the SBA distributes only through its disaster loan portal.

Questions

Is it really free?

Yes. The underlying forms are public U.S. government documents. There is no charge required, and you are free to copy and share the pack.

Are these the official forms?

They are editable Word and Excel versions we rebuilt from the official government PDFs so you can type and calculate. They mirror the questions and structure of each form, but the layout is approximate. Use them to prepare; complete and submit on the official form or portal — every source link is in the Readme.

Can I submit the Word or Excel file itself?

Usually no. Lenders and Grants.gov expect the official form — most of which are themselves fill-in PDFs or online portals. Treat these files as your working draft, then transfer your answers to the official form.

Do the Excel files calculate?

Yes. Totals, net worth and budget subtotals are built in. Cells with a light tint are for your entries; grey cells calculate automatically. Check each total against the official form’s instructions before you rely on it.

Does Momo Hero see anything I type into the forms?

No. You download blank files and fill them in on your own computer. Nothing is uploaded back.

Will the pack be updated?

We refresh it periodically and re-date the Readme. For anything time-sensitive, the source links in the Readme always point to the live official version.
